Continuing this string of corporate idiocy, check out this monthly bill that came though the mail. There is a major error on the outside of the envelope -- the FIRST thing people see and use to form an impression of the company they are doing business with.

Just for the record, someone internally threw a fit over this and the July bills were sent out without any text on the envelope, which is a major improvement.
